Our Team
About Kristen Bernockie, NP
Kristen Bernockie is a board-certified Family Nurse Practitioner and the founder of Eutierria Wellness, a primary care practice based in Hudson, NY. With over two decades of experience in healthcare, Kristen offers care that is deeply personal, grounded, and attuned to the whole person — body, mind, and spirit.
At the heart of Kristen’s practice is a commitment to slowing down, listening deeply, and meeting each patient exactly where they are. Her work blends the clinical precision of Western medicine with the insight and spaciousness of holistic healing, creating space for not just symptom relief, but meaningful transformation.
Whether managing chronic illness, offering preventive care, or guiding patients through life’s transitions, Kristen brings curiosity, clarity, and a profound respect for each person’s unique health journey.
Jaclyn Finn (AGNP)
Jaclyn is an Army Veteran, nurse practitioner (licensed healthcare provider) who prefers to use a holistic, science-based approach to diagnose, treat, and prevent disease. Holistic medicine focuses on identifying and addressing the root causes of illness, rather than simply managing symptoms.
Trained in both conventional medical sciences and natural therapies, Jaclyn prefers to blend modern diagnostics with a wide range of therapeutic modalities, including: clinical nutrition, herbal medicine, lifestyle counseling, physical medicine, Homeopathy, mind body techniques, and functional lab testing.
Holistic care is personalized, preventative, and patient-centered, with a strong emphasis on educating and empowering individuals to take an active role in their health.
Whether you’re dealing with chronic conditions, hormonal imbalances, digestive issues, fatigue, or simply seeking optimal wellness, our goal is to provide a safe, comprehensive approach to long-term healing and vitality.
Helen Rebekah Garber, FMP-BC
Helen Rebekah Garber is a board-certified Family Nurse Practitioner who believes that true wellness begins with compassionate relationships, thoughtful listening, and personalized care. Her approach focuses on treating the whole person – body, mind, and spirit – recognizing that physical health, emotional well-being, confidence, and life experiences are deeply connected.
Helen began her nursing career after training at LAC+USC Medical Center and spent seven years as a Neonatal Intensive Care Unit (NICU) nurse at Harbor-UCLA Medical Center and Providence Cedars-Sinai Tarzana Medical Center. She later earned her Master of Science in Nursing Education, served as a clinical nursing instructor at both Keck USC and LAC+USC Medical center, and completed her Family Nurse Practitioner training at the University of South Alabama.
Since becoming a Family Nurse Practitioner, Helen has cared for patients across the lifespan in primary care, urgent care, pediatrics, geriatrics, women’s health, and preventative medicine. She has a special interest in women’s wellness, menopause care, healthy aging, and helping patients create sustainable lifestyle changes that support lifelong health.
Helen also provides medical aesthetic services as part of her wellness philosophy. She believes that feeling confident in your appearance can positively influence self-esteem, emotional well-being, and overall quality of life. Her goal is to help patients look refreshed, feel confident, and embrace wellness from the inside out.
As a dedicated victim advocate with a Paralegal Certificate, Helen is passionate about supporting survivors of domestic violence and trauma through compassionate, trauma-informed care. She understands that healing often involves addressing both emotional and physical health, creating a safe environment where patients feel heard, respected, and empowered.
Outside of medicine, Helen has been a professional artist for over 30 years, bringing creativity, empathy, and attention to detail into every aspect of her practice.
Helen believes that the best healthcare happens in a smaller, more personal setting where patients are known, supported, and cared for as individuals. Her goal is to partner with each patient to promote lifelong wellness through prevention, education, evidence-based medicine, and compassionate care.

Amy Salvatore, Nutritional Therapy Practitioner (FNTP)
Amy is the founder of Naturally Rooted Nutrition & Wellness and is honored to be a part of the Eutierria Wellness team, where she collaborates with a group of compassionate professionals who share a passion for empowering others to take charge of their health.
With over 15 years of experience in the healthcare field, Amy has dedicated her career to helping individuals navigate their health journeys. She became a Nutritional Therapy Practitioner to deepen her knowledge and find answers to her own health challenges, to support others in a more personalized and holistic way. She understands firsthand how frustrating it can be to search for answers and feel unheard. She believes that every person deserves to have a trusted partner in their wellness journey – someone who listens, helps connect the dots, and provides guidance every step of the way.
Amy’s approach focuses on creating professional wellness plans tailored to everyone’s unique needs, goals, lifestyles and health history. Rather than relying on a one-sized-fits-all approach, she works collaboratively with clients to identify practical, sustainable strategies that support long-term health and help them feel their best.
As a mom, she understands the importance of creating a healthy, balanced life for both you and your family. She is passionate about helping busy individuals and families feel their best, have more energy and thrive in their everyday lives.
Drawing from her extensive healthcare background, and passion for holistic wellness, Amy takes a whole person’s approach to health, recognizing that factors such as digestion, sleep, stress, movement, nutrition and daily habits all play a vital role in overall well-being.
In her spare time, Amy enjoys caring for her small farm, where is raises goats, chickens, pigs, and sheep. Her love of natural living inspired her to create a line of handcrafted goat milk soaps, and other natural products, reflecting her belief that wellness extends beyond nutrition and into the choices we make every day.
Amy’s mission is simple: to help clients feel empowered, supported, and confident in their ability to make meaningful changes that lead to lasting wellness.

Michelle Santos, Receptionist
A mom of three beautiful girls, and a nana to five grandchildren. I have been in the medical field as a receptionist for over 20 years. I have worked at CMH in a few different offices, and I have been with Hurley Ave. Family Medicine for close to seven years and have ventured to Eutierria Wellness for the past two years working closely with Kristen Bernockie and Jaclyn Finn.
I love the approach that they have with their patients to not just jump to medicine, but to try and figure out the root of the problem and fix it that way. I believe in medicine, but don’t believe it’s the first and only answer.
I am blessed to be part of this team as we grow, I know that our patients will not only be educated while being genuinely cared for but will have a new understanding in “Health” care.
Blessings in Christ,
Michelle Santos
